Dementia Community: Supporting people with frailty and dementia

Emerging and increasing frailty often goes unidentified, and families living with dementia and frailty are missing vital opportunities to receive the right support at the right time. This session will highlight identification and care across acute and community settings, aligned with NHS priorities, specifically addressing how to:

• Recognise early signs of frailty
• Understand the interaction between frailty and dementia
• Support families and carers effectively
• Promote wellbeing and independence
